 If your product is defective and covered by our limited warranty, you should complete this form to receive our warranty service.

Please make sure you have read our Returns & Warranty Policy in full before completing this form. 

NOTE: Do not return products to VSLC until you have received instructions from VSLC. Products that are returned before your request is processed will either be returned to you at your own cost or destroyed.
